项目摘要

目的:職域において歯科保健活動を効果的に推進するために,集団歯科健診により基礎データを収集し,その成績に基づく集団健診と歯科相談室での保健管理からなる歯科保健プログラムを計画した。方法:集団健診は,某乳業社員1247名を対象として,歯の状態,歯周組織の状態,口腔清掃状態,顎関節の異常,軟組織疾患について行なわれた。また,質問紙法により歯科に関連する問題の有無,歯科保健に対する意識などについて調査した。成績および考察:齲蝕罹患傾向は高く,DMF者率は97.7%,DMFT指数は11.69であった。処置を必要とする者は40%に達し,特に20〜24歳で高かった。喪失歯所有者率は50.4%,一人平均喪失歯数は1.91と高く,特に35歳以上で顕著であった。歯周疾患の有病状態は,全年齢で高く,歯肉出血あるいはポケットを有する者の割合は94.2%,ポケット所有者率は67.8%を示した。この結果から,個人管理に重点をおいた保健プログラムの実施の必要性が示されたので,以下のように保健管理のプライオティーを設定した。1)受診者を要処置歯あるいは要補綴歯の有る者(C型),歯周疾患を有する者(B型),口腔清掃が不良の者(A型),歯科的問題のない者(O型)に分け,相談室では40歳未満のB型とA型を重点的に管理することが効率的である。C型および40歳以上のB型で深いポケット有する者は外部の診療所に委託する。2)相談室での活動として,予め計画した保健予防指針に基づく口腔衛生指導とともに必要に応じて中間診査を行なう。3)新入社員健診,および節目年齢による健診システムを導入する。4)転勤等も考慮し,関連事業所との連係を保健プログラムに組み入れる。今後は,個人の歯科保健管理の徹底をはかり,次回の集団健診をにより本歯科保健プログラムの効果を評価し,効率的な職域歯科保健システムについて検討する予定である。
