项目摘要

質問紙調査により自覚を主とした歯科保健情報を集め,それを基に個人および集団のための新しい歯科健康指標の作成を試みた。資料は,1992年より継続している某企業従業員約1,300人を対象とした歯科保健活動の自記式質問紙調査結果である。指標に用いた質問項目は、う蝕の自覚,歯肉異常の自覚,咀嚼障害,発音障害,外観上の問題,過去半年間の歯科的問題の6項目である。健康指標の作成には次に4条件を満たすように考慮した。(1)個人的にも集団的にも利用可能なこと,(2)指標は10〜100の範囲で,高い値ほど良好な状態を示すこと,(3)分布状態が正規分布に近いこと,(4)各項目の回答結果が指標に明確に反映することである。健康指標は,次の手順で作成した。(1)歯科的問題度の重要度に応じ,質問項目のウエイトを決定。ウエイトは歯科的健康度の自己評価点を基に年齢階級別に重回帰分析により検討。(2)各項目内回答カテゴリーに与える得点の変動パターンをカテゴリー変数の関数型で設定。得点の変動パターンの設定の客観的裏付けとして,各項目の回答度数分布状態に基づく情報量を用いて検討。(3)各項目の得点分布は山が左に寄った右に裾をひく分布になったので,各項目ごとの得点を項目ごとにベキ変換し,分布を正規分布に近似させた。その際,良好な状態ほど得点が高くなるようにした。(4)個人の歯科健康指標は,各項目の得点の合計値とした。以上の方法で計算した健康指標は年齢階級により多少の差が認められたが正規分布に近い分布形を示し,かつ集団検診の調査所見(DMF歯数,欠損歯数,歯周ポケット歯数など)と比較しても妥当性を裏付ける結果が得られた。さらに,歯科保健活動(歯科健康相談など)により経年的に本指標が改善されることが確認された。このように質問紙の回答から個人ごとの総合的な歯科健康指標(リスクアセスメント)を提示でき,それに基づいて集団としての歯科健康度も評価できることが示唆された。
