本研究では医学判断学における手法を応用し,一般成人や歯科医療関係者等の口腔保健や歯科医療に対する効用の評価を試みることを目的とした。調査は,予防処置参加者の中から16〜70歳までの男女180人を対象として質問紙を用いて面接形式で行った。質問紙では評点尺度法に準じた設問を作成し,歯の状態,歯肉の状態等について個人の効用値を求め,口腔調査所見との比較も行った。また,ほぼ同様の調査を歯科衛生学科学生87人についても実施した。一方,歯科領域の効用値の評価法を検討するため,予備的調査として歯科医療関係者など(20人)に対して評点尺度法と同時に,基準的賭け法と時間得失法による評価も行った。その結果,予防処置参加者の場合,(1)歯の状態としてう蝕歯,処置歯について検討したが,順に(1)臼歯金属冠(0.66)>(2)前歯前装冠(0.64)>(3)臼歯う蝕(0.39)>(4)前歯う蝕(0.37)となり,審美性を考慮してか前歯の効用値が低い傾向があった。(2)歯肉の状態では,(1)歯間部歯肉退縮状態(0.66)>(2)歯肉発赤腫脹状態(0.47)>(3)歯根露出状態(0.24)>(4)歯の動揺(0.23)となった。(3)口腔の状態としては,(1)噛みにくい状態(0.47)>(2)しゃべりにくい状態(0.41)>(3)歯の抜けた状態(0.37)>(4)歯が動く状態(0.30)>(5)歯が痛い状態(0.25)の順となり,機能障害よりも痛みに対する効用値が低いという結果であった。学生の場合,(1)歯の状態では,臼歯ではインレー修復(0.71),前歯ではレジン修復(0.64)が最も高く,臼歯,前歯とも抜髄が必要なう蝕(0.19,0.18)が最低であった。(2)歯肉では歯肉発赤腫脹状態(0.58)が最も高く,最低は歯の動揺(0.12)であった。予備的調査では,評点尺度法と時間得失法を比較すると後者の方が効用値の差が顕著であった。しかし,調査の効率性は悪いが歯を単位として,その生存を基準的賭け法で調査する方法が信頼性が高いと考えられた。

The purpose of this study is to evaluate the use of medical judgment in oral health care and dental care in general The survey was conducted in the form of face-to-face interviews with 180 male and female participants aged 16 to 70. Question paper is used to evaluate the standard method of setting questions, tooth state, tooth state, etc. to find out the individual's value, oral accent to find out the comparison. 87 dental hygiene students participated in the same survey. As a result, to prevent the treatment of participants,(1) dental state and dental erosion, treatment of dental decay, in the order of (1) dental metal crown (0.66)>(2) anterior dental crown (0.64)>(3) dental erosion (0.39)>(4) anterior dental erosion (0.37), aesthetic consideration, anterior dental decay, low tendency to use. (2)The order of the state of tooth flesh was: (1) state of tooth flesh retraction (0.66)>(2) state of tooth flesh swelling (0.47)>(3) state of tooth root exposure (0.24)>(4) state of tooth movement (0.23). (3)Oral cavity status is reversed,(1) Dental cavity status (0.47)>(2) Dental cavity status (0.41)>(3) Dental cavity status (0.37)>(4) Dental cavity status (0.30)>(5) Dental cavity status (0.25). For students,(1) the tooth state is the highest, the tooth restoration is the highest (0.71), the tooth restoration is the lowest (0.64), the tooth restoration is the lowest (0.19, 0.18). (2)Dentistry swelling status (0.58) is highest, and dentistry swelling status (0.12) is lowest. A preliminary survey was conducted to evaluate the difference between the time scale method and the time gain method. The investigation method is highly reliable.
