本研究は、『社会生活基本調査報告』の1976・1981・1986年と『全国消費実態調査報告』の1974・1979・1984年等の資料を使い、65歳以上の高年齢者の医療・食料・住宅・衣類等の月平均支出額と、その年齢層の男・女さらにそれぞれの勤労者・無業者の日常生活の活動(労働・睡眠・通院・家事および育児・テレビやラジオ)に費やす時間を分析している。本研究の計量分析の推定結果は、65歳以上の高年齢者の世帯において、医療の価格弾力性(医療の価格が1%増加すると、消費は何%変化するかという数値)はー0.655で極めて非弾力的である。つまり、医療費の高騰により彼らの医療費の支出額は必然的に増加する。一方、医療に対する所得弾力性(所得が1%増加すると、消費は何%変化するかという数値)は3.313であり、これを昨年度の研究から得られた25歳〜39歳の世帯のそれ(2.29)と比較するとかなり大きいことがわかる。結果、現状においては経済成長につれて高年齢者への公的医療費負担の高騰は避けられないことがわかる。65歳以上の高年齢者の世帯における医療需要と機会費用の関係は、夫婦共働きの世帯と夫婦共に無業者の世帯を比較した場合、前者ではー0.454(男性)とー1.162(女性)に対して後者はー1.370(男子)とー2.087(女子)と数値は大きく異なっている。つまり、賃金の上昇は、夫婦共に無業者の世帯においては大きく医療需要を減少させ、その支出も共に減少する。このことは、(賃金が高いために)労働市場に高年齢者が留まることにより、高年齢者への公的医療費補助の削減となることを示している。また、賃金の増加は特に高年齢者の女性の病院に通う頻度を減少させる一方、所得の増加は男女共にその頻度を増やすという計量結果が得られた。賃金と病院利用頻度の負の関係から、医療サ-ビスを勤労者に低い機会費用で供給することが今後の医療システムの重要な課題であるが、今後の公的医療費補助という財政負担の問題で高年齢者の労働と医療需要は大切な関わりをもっていると言える。
