精神分裂病の患者家族の心理的状態を把握するため、心理テストの実施と、半構造的な家族面接を行うことを今回の研究の方法として取り上げた。心理テストとしては,最初,Voughn & LeffによるExpressed Emotion(EE)研究で使用されたCamberwell Family Interview(CFI)を適用すべく試行を行った。この家族面接法はすでにその妥当性、信頼性において確立された評価を得ており、現在あるものとしては最も有用と思われたので選択したものであるが、一人の家族成員の面接に要する時間が長く、面接後の評価も評価者の訓練と経験を必要としており、実際に使用するには複数の協力研究者を要するため、今回の研究の臨床場面での使用に適さない物であった。CFIの短縮版あるいは簡易法を今後開発し、実際の臨床の場でも容易に家族のEEを測定できるようになれば、患者の予後の改善、患者家族への心理教育的アプローチに非常に役に立つと思われるが、これにはさらなる研究の蓄積が必要とあると思われる。当面はすでにある心理テストバッテリ-のMMPIなどを、家族にも適用することも検討したが、被検者になることへの家族の抵抗が大きく、治療関係への影響を考えるとあまり望ましい物ではなかった。家族の一人が精神分裂病に罹患した事が、家族の生活や家族関係にどの様な影響・変化をもたらしたかと言うことに焦点を当てて、家族面接を行った。この際には、これまでの家族の歴史や関係の成り立ちを視覚的に家族自身が理解できるようにと、家系図作成などの家族療法的な技法を取り入れた。個々の症例では家族背景や家族が患者の病状にどの様な影響を与えているかを治療者が理解する上で、貴重な知見が得られた。また患者家族にとっても、従来の家族教室など集団的なアプローチより、より有効な精神的援助となったと思われる。今後がこのような治療的・心理教育的な性格を持った家族面接を、治療施設のシステムの中でどの様に位置付けて行くか、またこのような家族面接の有効性をどの様に評価して行くかを、引き続き検討して生きたい。
