ハイリスク児に対する超早期からの発達援助・教育に関する研究について、当初の研究計画に沿い、以下2点について研究実績が得られた。1.NICUにおけるチーム・アプローチに関する研究:NICUに入院する骨形成不全症候群の1症例について、患児とその家族に対し、臨床心理学、小児科学、看護学によるチームを組み、約10カ月にわたり子どもに対する情緒・行動の発達援助および家族に対する障害受容、愛着形成のための心理学的援助・介入を行った結果、超早期における母子相互作用の促進が可能となった。また、チーム・アプローチの成果として次の点が明らかにされた。まずNICUにおける心理援助者の機能は、(1)患児の家族を情緒的に支えること、(2)患児と家族の早期の愛着形成を援助すること、(3)直接的、間接的に患児の心理、情緒的発達を援助すること、(4)家族と医療スタッフを繋ぐパイプ役となること、(5)医療スタッフを精神的に支え、医療スタッフの燃えつき現象を予防し、適切な医療的ケアが行えるよう配慮すること、の5点にまとめることができた。更に医療スタッフは、(1)両親の生育歴、性格、心の動きを知り、家族理解を深めることにより家族との信頼関係を促進することができ、(2)心理援助者を通して、患児への医療的ケアの評価を受けることにより、より積極的に患児のケアにかかわることが可能となった。なお、この研究成果の詳細は、第15回視覚障害乳幼児研究大会(1993.8.21,名古屋)にて発表した。2.極小未熟児のフォローアップ研究:小児科外来未熟児健診において、極小未熟児の母子を対象に心理治療的介入を行ったところ70例中48例に、子どもの行動あるいは母子関係上の問題が認められた。子どもの行動の問題は、主に多動傾向と衝動コントロールの悪さであった。これらの問題行動および母子関係の問題は心理治療的介入の継続により改善をみた。なお、この研究成果の詳細は、乳幼児医学・心理学研究2巻1号(1993.11)に発表した。
