研究対象および方法今年度行った研究対象としては,鶴見大学歯学部附属病院に来院した患者の中から、X線診査で根尖周囲に小指頭大以上の透過像を有する症例を選択した.各症例は術前にMRI検査を実施してから,治療を行った.外科的処置の適応になった症例は,術中に病変部を摘出し病理組織的検討も行った.術後一定期間経過した症例は再度MRI検査を行った.以上のようにして各症例の臨床診断・X線画像診断・MR画像診断・病理診断を比較検討した.結果及び考察今回被験対象とした症例のうちX線画像診断とMR画像診断が一致しなかったのは症例が半数近くあった.すなわちX線写真上では病変の範囲が患歯周辺に限局されていると判断される症例でも,MR画像上では数歯にわたって骨髄信号が高くなり、炎症の存在が疑われる場合や,X線写真上で境界明瞭な嚢胞状の像を呈していても,MR画像では肉芽腫と判定された症例があった.MR画像診断と病理診断は,外科的処置を行った症例中で高い確率で一致していた.一致しなかった1症例は嚢胞に移行する途中の慢性病変と考えられるが、病理組織的観察では上皮組織の不完全な形成が一部認められていた。この症例はMR画像ではT1・T2・プロトンいずれの画像でも低信号を示し肉芽腫と診断されていた.また本法は多様な角度から撮影が可能であるので,矢状方向・水平方向の断層撮影を行い,3次元的な病変の広がりをかなり的確に予測することができ,外科処置に際して有効な情報が得られたと思われる.以上のように根尖部病変に対するMRIの応用は,臨床診断にさらに新しい診断基準をもたらす可能性が示唆された.今後さらに症例を増やし検討を続ける所存である.
