平成7年10月、本調査研究に関して“大学附属病院等における卒後臨床研修について"そて中間まとめを行った。その内容の大筋を示す。1.卒後臨床研修について(1)卒後臨床研修の考え方:卒後教育及び医師国家試験を経て、期待される医師像を見直し、幅広い基本的診療能力を修得させるために行われるものであり、よき医師を養成する上で不可欠である。しかし、その実施に当たっては1)幅広い基本的診療能力の修復 2)臨床研修施設の独創性 3)研修医の進路を考えた多様性、選択制等が必要である。(2)大学病院における卒後臨床研修の課題1)病院全体としての研修プログラムを作成 2)研修方式はロ-テイト方式を採用する。 3)研修プログラムの公開と研修医の選択制 4)研修実施体制と評価体制の確率2.全国レベルでの卒後臨床研修改善・充実に向けた組織の設置大学病院関係者及び臨床研修指定病院関係者等による独立した組織を設け、以下の事業を行う。(1)到達目標、研修プログラム、研修方式、研修実施、評価体制等の調査研究並びに相互評価(2)大学病院と臨床研修施設との連携促進3.大学病院における卒後臨床研修改善・充実の取組(1)研修目標:平成元年の医学関係者審議会議臨床研修部会作成の「卒後臨床研修目標」を参考にして、各病院独自の基本的、総合的研修目標を定める(2)研修プログラム:病院全体の研修プログラムを作成-自主性、多様性、選択制を確保する。(3)研修実施体制:病院長を研修の最高責任者として卒後研修委員会を設置し、実施体制及び責任体制を確率する。研修医の受け入れ数は指導体制、特に指導医の資格と人数に基づいて定める。(4)研修修了の評価:評価方法、体制の確率が必要である。 などである。以上の中間まとめを行い、今後は卒後臨床研修の抜本的改善、充実の必要性が認められることは、各専門家間に於いても同一であり、それを施行する上での方策等の不一致を議論し、一日も早い具体案を作成する必要がある。そのため文部・厚生両省を含めて大学関係者、臨床研修指定病院、医師会の方々が同一の場で議論し、合意点に到達すべきと思われる。
