Assessment of pathophysiologic diagnosis and quantitative evaluation of therapeutic effect in cardiac . failure has been investigated by using myocardial metabolic and receptor imaging from 1995 to 1998. The following researches were done from the basic and clinical points.1. basic research using isolated perfused rat heart and autoradiographyMetabolism of ^<123>I-BMIPP was clarified by isolated perfused rat heart model. Glucose uptake in stunned myocardium was also clarified by ^<31>P-MR spectroscopy and it was confirmed to be caused by the translocation of glucose transporter. Tracer kinetics of ^<99M>Tc-nitroimidazole was also clarified. These tracers were applied to animal models such as DCM hamster (Bio l4_<beta>6) etc. It was conformed that the metabolic and receptor changes were preceded before the flow and functional changes by autoradiography.2. clinical research using SPECT and PET^<123>I-BMiPP myocardial SPECT was useful for the assessment of severity of hypertrophic cardiomyopathy (HCMz) and therapeutic effect of Coenzyme Q1O in dilated cardiomyopathy (DCM). ^<123>I-MIBG was useful for the severity and therapeutic effect of BETA-blocker in DCM.Furthermore, ^<18>F-FDG myocardial PET was extremely useful for the pathophysiologic diagnosis of HCM and DCM.These findings should be necessary for the indication of heart transportation.
