上顎無歯顎患者を被験者とし,Wilhelmら(1992年)の方法に準じてHAAGテンションゲージ(ジャモス)を用いて,上顎総義歯の維持力を計測し,測定方法の妥当性および患者の上顎義歯の維持安定性に対する主観的評価と維持力測定値との関連性について検討し,以下の結果を得た.1.Wilhelmらの報告した維持力計測方法は,上顎中切歯舌側面にテンションゲージを用いて回転力を加え,上顎義歯が後方部から離脱した時の力を維持力とするものであるが,上顎総義歯の前方弓状部の床翼の長さに影響を受けるため,本研究ではこの計測とともに,上顎総義歯の上唇小帯切痕に下方向への力を加えたときの離脱力も同時に計測した.これらの計測方法は,測定のための特別な義歯調整もいらず,短時間に計測が終了できるため患者の負担も少なく,また測定値のばらつきも±20N程度であることから,臨床サイドでの維持力の評価方法としてかなり有用な方法であると考えられた.2.各被験者の計測値は,義歯の咬合状態・計測直前のかみしめの程度・測定時の開口量によって影響を受けるため,計測誤差も含めた測定値のばらつきは認められたが,平均値とともに計測値の範囲を比較することにより,被験者間の比較は可能であると考えられた.3.各被験者の維持力は,100N以下から1000N以上までと様々であり,同一被験者における2つの計測方法による差も認められた.現義歯の維持力に対して患者自信が不満を持つ症例では,その維持力は200N以下であったが,1000N以上の大きな維持力を示した症例においても‘強すぎる'と判断する患者は認められなかった.また旧義歯と新義歯,リベース前後の維持力の変化を数値として比較することも十分可能であった.
