今年度は、各眼疾患に対する硝子体手術後の水晶体の変化、特に核白内障の発生頻度ならびに核白内障を促進させる因子の検索を目的として研究を行った。過去5年間に、当病院眼科で行った硝子体手術症例のうち、術後1年目の水晶体所見が明らかな有水晶体眼症例215例236眼を対象とした。各症例の術後1年の時点における核白内障発生に及ぼす20因子ついて、単変量解析さらにロジスティック回帰分析による多変量解析を行った。その結果、従来言われていたように、高年齢は術後の核白内障発生に有意に関与していた(P<0.000001)。年齢の他に、核白内障を抑制させる因子として原疾患(糖尿病網膜症)があり、またガスならびにシリコンオイルタンポテ-デは核白内障発生を有意に促進していた。術前の硝子体出血、網膜剥離、手術回数などは核白内障発生に関与していなかった。以上の結果から、年齢が核白内障発生に大きく関わることに加えて、糖尿病網膜症患者ではその発生が抑制されることが推察された。さらに、眼内タンポナ-デの有無も核白内障の発生促進に関与していることがわかった。これらのデータは硝子体・白内障同時手術を選択する場合の資料となると思われた。
