Actinobacillus actinomycetemcomitans, Porphyromonas gingivalis and Bacteroides forsythus have been considered as causative organisms of periodontal disease. The aim of this research project was to evaluate the disease activity by using a polymerase chain reaction (PCR) based method for detection and quantification of these periodontopathogens and examine the relationship between the presence of these pathogens alone or in combination and the effect of the initial periodontal therapy. Subgingival plaque samples were collected from the periodontal pockets of patients with moderate periodontitis at baseline and after initial therapy. The bleeding on probing (BOP) was chosen as clinical parameter. In the periodontal site where these three bacteria co-exsist and n the site where A. actinomycetemcomitans exists with either P. gingivalis or B. forsythus, BOP trends to remain after the initial therapy. In proportion as the number of A. actinomycetemcomitans cells increases at baseline, BOP also trends to remain after the initial therapy. The increase of the number of A. actinomycetemcomitans cells at baseline is strongly related to the frequency of simultaneous detection three bacteria after the initial therapy. BOP after the initial therapy were observed with considerable frequency in the site where these three bacteria co-exsist. These results suggest that the quantification of A. actinomycetemcomitans at baseline and the detction of P. gingivalis and B. forsythus are available to predict the effect of the initial therapy and comprehend the periodontal disease activity.
