项目摘要

1.研究の経緯遺伝医療が進展する中,看護職にも「遺伝」との対応が予測される今日,看護教育や看護実践において早急に「遺伝医療」への充実を図ることが必須であると考えられ,本研究は臨床や地域で活躍できる遺伝看護の専門家育成のための教育モデルを提案することを目的として行われている.一昨年の研究結果では,看護基礎教育は遺伝医療を見据えたかたちでの意図的なカリキュラム編成が行われていなかった.また,昨年の結果は,臨床看護職が日常業務の中で遺伝に関する事象に遭遇する頻度は高くなっているものの,「遺伝」に関する知識が少ないと認識していた.また,遺伝相談などの専門部門への参入状況は極めて少なく,遺伝専門職として系統的に基礎および卒後教育をプログラムしていくことの必要性が示唆された.本年度の本研究計画では,これらを踏まえながら,教育モデルを作成することであったが,遺伝医療全体における看護の研究的状況が不明瞭であることと,諸外国の遺伝医療における看護職の機能についての検討が不十分であると考えられたため,前者の遺伝に関する研究的状況について,過去10年間の検討を行うこととした.2.研究実施内容目的:わが国の遺伝医療全般と看護領域における「遺伝」への関心およびその研究について量的および質的内容を把握し,過去10年間の遺伝医療の変化について把握する.方法:遺伝に関するキーワードを設定し,医学中央雑誌より過去10年間の文献検索を行った.各年代における文献数を医学全般と看護領域に分け統計処理を行うと共に,看護領域に関してはその内容的検討を行ない,研究的推移を把握した.3.研究結果医学界全般の研究推移は,1994年以降大幅にその文献数の増加が認められたが,看護領域においては10年間の文献数の増加は殆ど認められなかった.看護領域における文献で中核をなしているものは,小児・母性領域のものが多く,その中でも「出生前診断」に関する研究が最も多かった.「遺伝相談」「遺伝カウンセリング」などの研究は,地域保健領域での研究が多く,相談業務の現状やその内容分析,事例報告といったものであった.4.考察遺伝医療は近年急速な進歩を遂げているものの,「遺伝」への看護職の研究的志向は極めて薄かった.これは,現状の看護教育に帰するところが大きいのではないかと考えられた.
