今年度は、日本における看護婦の専門職的自律性の概念分析を行った。Rodgersによる概念分析方法を用い、医学中央雑誌より1987年から2001年の文献について、看護婦の「自律性」「主体性」をキーワードに検索を行った。この結果、「自律性」については計25件、うち原著論文は10件、自律性がテーマとして取り上げられていた時期は1994年以降であった。「主体性」は1987年以降、計37件、うち原著論文は7件のみであった。専門職的自律性の前提としては、看護の専門家としての「能力」が自律性に必須の基盤となり、それを支える基礎教育と継続教育が必要条件であった。また、「自律性」と非常に近接する概念として「主体性」が「自律性」と混同されながら用いられているものの、2つの概念には分析の結果、差異が示された。「主体性」は看護者が主体となり、個人として判断し行動することが主に言及されていたが、「自律性」は個人としての特性を言及するだけではなく、看護という職業の「自己決定」「統制力」「倫理観」「責任」まで言及されていた。従って、「自律性」は看護の内的側面に関する「専門性」「独自性」「能力」という概念と外的側面と関連を持つ「権威」「尊重」「法律」「協働」「伝統的役割」という概念との関連性が示された。概念分析の結果は、Ballou(1998)によるアメリカにおける概念分析の結果との大きな差異は示されなかった。この結果により、概念モデルを構築し、専門職的自律性を測定する項目を抽出した。この妥当性について、看護学研究者と看護管理者とともに検討を行い、「責任」「医師との協働」「医療訴訟」に関する項目について一致に至らず、項目の洗練を要した結果、38項目が抽出された。この質問項目について10名の看護婦に調査を行い、更に内容妥当性を検証し、「自己統制力」「医師との協働」に関する項目の修正を行った。
