项目摘要

看護管理者46名を対象とし「看護実践における倫理的問題」について調査した結果、「看護師-医師の関係における対立」「インフォームドコンセント」「看護師の充足状況」についての倫理的問題を頻繁に体験し、悩んでいることが明らかとなり、既に実施したスタッフナースの調査と同様の結果を得た。アメリカの結果と比較すると、「看護師の充足状況」が問題であることは共通であったが、「延命に関する問題」が高く認識され、「インフォームドコンセント」「看護師-医師の関係における対立」は問題として低い認識であった。本研究では、多くの看護師が倫理的問題に悩んだ体験を持っていたが、自身の倫理に関する知識に自信がなく、自ら倫理的問題を解決するという看護師は僅かであり、看護師が倫理的問題解決に活用できるリソースが少ない状況であった。アメリカにおける看護師の患者擁護者としての活動について、ミネソタ大学にて、「患者擁護(アドボカシー)」に関わる看護師の実践について、看護師、研究者、教育者5名にインタビューを行った。この結果、看護倫理教育の充実により、患者の擁護者としての役割を看護師が認識し、実践に取り組んでいること、看護師が擁護者としての役割を果たすためには、看護師-患者間の関係を築くこと、医師をはじめとした医療チームにおける良好な人間関係を築くことが不可欠であった。以上の結果から、看護師の充足状況がアメリカよりも低く、医師との関係に悩む日本の看護師が患者の擁護者としてその役割を果たすことは非常に難しいということが示唆された。しかし、日本において看護師は実践における倫理的問題を認識し、その解決を図り患者の擁護者としての役割を果たす動機付けは高い。看護師が擁護者としてその役割をとるためには、看護師の倫理教育内容の洗練と、看護師個人の努力だけではなく、医療施設における組織的な取り組みが不可欠であることが明らかとなった。
