本研究では東京・神奈川・兵庫の8医療施設で研究承諾の得られた59名に面接調査を行なった。59名の対象者の内57名はすでに亡くなっておりその家族への面接調査となった。分析は質的研究方法の分析方法に沿って行なった。在宅医療を選択した対象者とその家族は、ターミナル期になり「治らない」ことがはっきりした時にそこからの転換をはかり、入院環境が対象者に与える影響を最も重要に考え、在宅医療を選択していた。家族は「在宅死」や「在宅での看取り」を在宅医療の目標としているのではなく、少しでも安寧に長く、よりよく生きることを目標としていた。対象者とその家族の在宅医療やHPNに関する知識はほとんどない状態で、"無知であり""初めての経験"であると述べていた。これに対して医療者からの適切なインフォームド・コンセントやコーディネート、教育・指導などの支援が提供されれば、病状への予測性と対処可能感、問題解決のコーピング能力も高まり、在宅医療への移行がスムーズ行なわれ、実施後の満足度も高かった。ターミナル期になった段階で家族が引き受けることが家族の役割と考え、それを果たすことが家族にとって大切なことだと考える家族は在宅医療の選択に関する意見の一致度も高かった。HPNに関する調剤や薬剤配送サービス、輸液ポンプの利用は在宅での医療処置の負担を軽減し、対象者の生活を広げるのに効果的であった。在宅医療で提供された訪問診療への満足度は高かった。しかし、対象者が医療処置や生活面でのセルフケアができる場合の訪問看護の評価は低かった。在宅医療のみの評価は高くても、在宅開始時のパフォーマンスステイタスやそれまでのインフォームド・コンセントが癌治療のプロセス全体での満足度に影響していた。費用に関しては保険適応内で実施されているため負担感は少なく満足していた。今後、在宅医療への情報やシステムが整備されることを望んでいた。
