患者は癌に罹患する前までは、自分のことは知っておきたい、正直に真実を伝えて欲しいと思っていても、どのように伝えて欲しいというのは意識せず、がん疾患や治療にしては正確な知識はあまりなく、漠然としたイメージによる不安と期待の中で、癌であることを確定診断の過程に沿って段階的に伝えられていた。癌であるとはっきり伝えられ、場合によっては、進行がんであることも伝えられていたが、治療の可能性があることと一緒に伝えられていた。多くの患者がショックを受けていたが、気持ちを切り替え、癌である事実を受け止め治療を考えるようになっていた。治療の意思決定に関しては、全て医師の言う通りではなく、自ら考えて治療の意思決定をしいきたいとしながらも、素人で専門的知識がないので、治療選択の判断は難しく、医師を信じ任せるしかないとの考えており、医師が信頼できるかを判断しながら、治療の意思決定に尚けての行動を起こしていた。再発や転移の段階では、これまでとは違い予後や治療効果の厳しさをある程度は意識し、迷いながらも進んでいくしかなく、治療を受ける意思決定を行っていた。治療を繰り返し行くなかで、現在、完治はしない状況で、これといった治療がないので様子を見る段階である事実を治療の結果として伝えられていた。この事実に対してショックを受け、これから先はどのようになるのか、やっぱり死ぬのかといった思いがよぎりながらも、やれることをやって、なるようにしかならないと、現実に淡々と向き合うようにしていた。看護師は、医師からは患者・.家族を精神的にサポートし、患者に関する情報の提供や患者との調整役、患者からは精神的に支え、身体的な苦痛の軽減や必要な情報の提供、医師との橋渡し役、家族からは患者の心身のケアや身の回り.の世話、患者の様子の情報提供や相談相手の役割りを期待されていると認識していた。悪い知らせを伝えられた患者の看護の方向性としては、患者も家族も辛い現実を受け止め、後悔のないよう前向きに、自分らしく有意義に進んでいくよう支援する、患者が自ら決断し、納得して治療を受けられるよう支援することをあげ、そのために(1)医師の説明後に援助が始まるが、医師と看護師が同じ目標を持って援助していく、(2)患者の性格やコーピングパターンを踏まえ、本心や辛い気持ちが言えるような関係作りをして、とにかく話を聞いていく、(3)患者の意思に沿った援助をするために、患者の意思を引き出し、これからのことを前向きに自らの意思で決定するよう関わる、(4)最期まで患者の苦痛を軽減し、患者と家族の時間をつくり、患者と家族が決めたことはとことん支えるといった援助を行っていた。
