项目摘要

1.摂食障害患者の治療施設全国計1,726ケ所のうち、平均受診数10名以上の施設、約270施設の担当医に対して、"摂食障害罹患同胞対患者"を対象とした摂食障害罹患感受性遺伝子研究への協力の調査を行なった。その結果、全国170ケ所以上の施設から協力可の回答が得られた。その内訳は50施設以上から、「現在罹患同胞対を診療しており、患者、家族の同意が得られるならば、研究に協力してもよい」という回答であった。さらに、別の110施設以上から「現在罹患同胞例は診ていないが、今後罹患同胞の診療の機会があれば同意のもと協力できる」旨回答があった。2.国の「ヒトゲノム・遺伝子研究に関する倫理指針」に基づいたインフォームドコンセントが必要であるため、患者・家族用説明書および同意書を作成し、国立精神・神経センター・倫理委員会に申請し、2001年8月に承認されたよって各施設でそれをそのまま使用、もしくは倫理委員会が設置されている施設では「迅速審査」等を経て、所定の手続きを得たうえで実施することが可能となった。未成年者が対象である場合も少なくないため人権保護については細心の注意を払っている。3.実際の試料収集にあたっては、病型、病歴など必要な診療情報を体系的に収集する必要があるため分担研究者間で検討し、手順をマニュアル化し、診断、試料の統一化を図った。4.今後、全国各診療施設で診断基準の徹底した統一的実施とそれに伴う問題点の改善、さらには精度の高い適用が課題となる。内容的には信頼度の高い診療情の承得や心理テスト等の完全な実施である。平成14年3月現在、約10組の"罹患同胞対"の血液試料が各施設から集まり、同様に弧発例については、国立精神・神経センター国府台病院の患者の約100例に加えて、約80例の血液試料が各施設より集まって来ている。罹患同胞対連鎖解析は国立国際医療センター(笹月健彦所長)で実施予定である。
