项目摘要

Objectives(1)To examine challenges of circumstances and difficulties in independent living for young adults with pediatric diseases, such as childhood cancer survivors, insulin dependent diabetes mellitus patients and congenital heart disease patients.(2)To establish guideline of social support for young adults with pediatric diseases and to make a guide book for adolescents with pediatric diseases to prepare for independent living.Procedure(1)Some questionnaires were made to examine challenges of circumstances and difficulties in independent living of young adults and social support. One questionnaire was for pediatricians, one for teachers of special schools for the health impaired, one for adult patients and the other for undergraduates.(2)Questionnaires were delivered for 1000 subjects.(3)Semi-constructed interviews with subjects who agreed with interviews were performed.(4) Symposiums on guideline of social support for young adults with pediatric diseases and working group to make a guide book for adolescents with pediatric diseases were held.Results(1)567 answers of questionnaires were collected. Adolescents and young adults with pediatric diseases had many challenges for proceeding to higher levels of education, employments and marriages. In some ways the images of independent living of young adults with pediatric diseases were significantly different from those of undergraduates.(2)Teachers of special schools for the health impaired supported less for young adults than pediatricians.(3)Information, which encourages young adults to ask for advice, should be provided for them before they think advice is needed.(4) The guideline of social support for adolescents and young adults with pediatric diseases and a guide book for adolescents with pediatric diseases to prepare for independent living were established. 目的(1)研究患有儿科疾病的年轻成年人独立生活的环境挑战和困难,如儿童癌症幸存者、胰岛素依赖型糖尿病患者和先天性心脏病患者。(2)制定青少年儿科疾病社会支持指南,制作青少年儿科疾病准备独立生活指南。程序(1)对青年独立生活的环境挑战和困难以及社会支持进行了问卷调查。一份问卷是针对儿科医生的,一份是针对健康受损特殊学校教师的,一份是针对成年患者的,另一份是针对本科生的。(2)共发放问卷1000份。(3)对同意访谈的被试进行半建构访谈。(4)召开青少年儿科疾病社会支持指南专题讨论会和青少年儿科疾病指南编写工作组。结果(1)共回收问卷567份。患有儿科疾病的青少年和青年在接受更高水平的教育、就业和结婚方面面临许多挑战。在某些方面,患有儿科疾病的青年成人与大学生的独立生活形象存在显著差异。(2)健康受损特殊学校教师对青壮年的支持少于儿科医生。(3)在年轻人认为需要建议之前,应该向他们提供鼓励他们寻求建议的信息。(4)制定了《青少年和青壮年儿科疾病社会支持指南》和《青少年儿科疾病独立生活准备指南》。这些文档可以从http://homepage1.nifty.com/k_tanigawa/carryover.html下载。
