大学病院における慢性疾患専門外来での看護師の役割の明確化のため、患者の自己管理を支援するための患者指導や看護援助の実態を把握することを目的に52施設、110名の回答を得たアンケート調査と先進的な取り組みを行っている7施設の外来看護師に面接調査を行った。1.アンケート調査1)個別指導:看護師が個別指導で心がけていることとしては、<患者の状況を把握して関わる><患者の思いを聞き受け止める><患者が思いを言える関わり><患者が目標を持てるように>などがあげられた。外来での個別指導における問題として、198の問題が挙げられ、<コスト・保険診療の仕組みの問題><個別指導を行う時間がない><看護師の指導能力の不足>などに整理できた。2)集団指導:看護師は集団指導で心がけていることとしては、<指導能力の向上、体制の整備><指導内容を理解してもらう工夫><教室をともに作り上げる><多様な生活状況><集団指導後のフィードバック>があげられた。外来での集団指導における問題として、53の問題が挙げられ、<構成・内容の再検討が必要><指導時間の長さの不適切><他の業務との調整が困難><募集方法の問題>などに整理できた。2.面接調査1)先進的な取り組みを行っている外来では、外来と病棟を一元化、看護相談室の設置、認定看護師など専任看護師の配置、センター方式での診療システムなど外来での患者指導が実施できる体制を整備していた。2)看護師は外来指導による収益を数字で示したり、患者指導による効果を実績として示すなどさらによりよい体制を整備するために取り組みをおこなっていた。以上の結果より、大学病院における慢性疾患専門外来の患者指導を行える体制としては、問題点も多いが、その中でも外来看護のあり方を摸索し、よりよい体制が整えられるよう看護職者として整備・改革を行っていくことが現在の外来での看護師の大きな役割であると言える。
