本研究の目的は、精神科外来における看護相談(看護職による相談活動)に焦点を当て、精神科外来における看護職の相談活動について明らかにすることである。そこで今年度は、精神科外来における看護職の相談活動が明らかになるような独自の調査用紙を作成し、質問紙調査を行なった。その主たる結果である看護職の相談活動や指導については、以下のように整理された。1.外来で看護職が相談活動や指導を行なっているのは、回答のあった施設のうちの48.7%であった。2.相談活動や指導の開始は、多い順に、患者や家族の要請により開始、看護職が必要性を判断して開始、医師からの指示や他部署の職員の要請による開始であった。3.相談活動や指導の内容は、多い順に、服薬に関して、病気・病状・症状に関して、日常生活・生活の仕方・過ごし方に関して、受診・通院・入院に関して、家族の相談や教育に関して、資源に関して、対人関係に関して・電話相談に関して、専門職間の連携に関してであった。4.相談活動や指導の現状は、1)人員配置の問題から時間的に余裕がない、2)専用の記録用紙がないことや最低限の記録しか残せない状況などにより継続性がない、3)患者さんへのプライバシーの保護ができていない、4)他職種との共同・協力・連携で何とか行なっているということに整理された。5.相談活動や指導上の困難さは、1)人員・場所・時間の確保が困難であること、2)実施内容の評価の困難さ、3)他職種との連携・情報交換不足に整理された。6.今後の提言や意見としては、1)看護職のみならず他職種をまじえた相談窓口の必要性、2)情報伝達の必要性、3)記録を残すことの有用性、4)責任範囲を明確にする必要性、5)サービスで行なっていることを評価していくこと(診療報酬への加算)に整理された。
