项目摘要

今回の調査で、苦情解決制度の活動実態、苦情解決制度の課題、施設側が受けとめた苦情の姿、家族への期待等が明らかになった。苦情内容については、最も多かった「職員の言動・態度について」とともに、「その他」のなかで「ケアの内容、施設での医療のあり方等」に対する苦情が多いことに注目できる。苦情を分析してみると、日常的なケアの質に関わる項目がそれほど多くないことがわかった。それは、苦情の言いやすい項目、情報の得やすい項目に集中していると理解できる。今後は、利用者、家族の思いの把握とともに、ケアの質を高めるための情報開示も視野に入れた制度構築が望まれる。苦情受付体制に関しては、「利用者に痴呆老人が多いため、介護内容等の苦情が表れにくい」という回答や、「何を苦情として捉えるのかが難しい。要望や意見は多数頂くものの、話し合いや施設内での検討で解決できてしまうものが多い。そのような場合は、現在の段階で、苦情ではなく、要望として処理しているため、件数としては少ない。」という回答があるように、苦情を単純に数で評価できない側面もある。しかし、苦情を正確に記録し、利用者・家族の声に真摯に対応しようとしている施設側の姿勢は感じ取る必要があろう。苦情解決制度の課題としては、「苦情や不満が表面化しにくい」「苦情を出しやすい体制ができていない」との意見にもあり、「苦情を言える関係」が根本にある。そして、苦情を言える関係を見たときには、利用者と援助者の関係とともに、利用者の代弁者としての家族、さらに利用者の精神的支えも含めたケアのパートナーとしての家族の役割を検討する余地がある。事業者のなかには、寄せられた苦情に対してどのように対応したか、そのプロセルや対応策を記録に残し、情報開示している施設もでてきている。苦情を受け止める仕組みが整備された次の段階として、苦情解決のプロセスや対応結果(申し立て者とのフィードバックを含めて)を開示していくことが利用者・家族の信頼を高めることにつながるであろう。
