项目摘要

本研究では組織的な視点から医療事故防止対策、特に注射に関する事故防止を検討するために,第一に業務分析から業務過程に組み込まれている防護内容を明らかにする、第二に事故事例分析から防護の機能不全に影響を与えた潜在的条件を明らかにする、第三にエラー誘発条件の中でも最も重要といわれる作業現場要因とエラー発生との関連性について検討することを目的とした。調査対象はエラー事例の報告システムが整っている4病院14病棟とし、さらにその病棟に所属する看護師とした。第一の業務分析の結果、基本の注射業務過程には8つの業務段階があり、この業務過程は単純化,標準化,可視性,可逆性に乏しくエラーが発生しやすい状況であった。エラー防止のための防護は多重に配置されておらず,一つのエラーが事故に結びつきやすかった。第二の潜在的条件に関する結果では、エラーの誘発条件は多種存在したが,それ単独では事故は発生せず,その背後には注射業務過程や防護内容の欠陥が存在していた。第三のエラー発生に関連する作業現場要因の結果は、看護師が働く勤務帯によって関連要因が異なっていたが、総じて多忙の存在や経験年数が短い看護師の場合に有意にエラーが発生していた。これらはエラーを誘発させる要因として,改善策を検討する必要があった。また、勤務前疲労感が低い場合に有意にエラーが発生したとの回答があり、これらはエラーの発見を促進させる要因とも考えられた。事故発生の背景には業務過程の複雑さ、防護の欠陥、エラーを誘発させる潜在的条件の存在などが複雑に絡み合っており、医療従事者個人への注意喚起のみでは事故防止は不可能である。そのため、事故を防止するためには業務過程や防護内容の欠陥を解明し,防護の強化や作業現場要因等の潜在的条件を改善し、さらにはエラーを未然に発見できる要因も検討するなど,複数の側面からの組織的なアプローチが重要であった。

This study focuses on the organizational perspectives of accident prevention strategies, special injection related accident prevention issues, first business analysis, business process analysis, protection content analysis, second accident case analysis, protection functional inadequacies, and potential conditions analysis. The third is the most important factor in the job site. The investigation was conducted in 4 hospitals, 14 hospitals and 14 nurses. The results of the first business analysis, basic injection business process, eight business stages, the business process purification, standardization, visibility, reversibility, lack of development and status. To prevent accidents, multiple configurations are required. The second potential condition is related to the existence of multiple inducing conditions, such as single accident occurrence, injection business process and protection content. The third is the occurrence of a job site factor that results in the presence of a caregiver who is working in a different service area, the presence of a caregiver who is busy, the number of years of service, and the occurrence of a caregiver who is intentionally working in a different service area. The main reason for this is to improve the policy. The answer to this question is intentional, and the answer to this question is important. The background of the accident is the complex of business processes, the lack of protection, the existence of potential conditions, the complex of networks, the personal attention of medical personnel, and the impossibility of accident prevention. The business process, the prevention of accidents, the lack of understanding of protection content, the enhancement of protection, the potential conditions of job site factors, etc., are improved, the prevention of accidents, the investigation of key factors, and the importance of the organization of multiple bottom surfaces.
