项目摘要

チーム活動の実態が事故やエラー発生とどのような関係にあるのかを明らかにするために、本年度は以下の研究をおこなった。1.医療従事者の業務内容及びエラー発生に関する研究230名の看護師に量的調査結果を分析し、注射業務エラー防止のためのチームメンバー間のコミュニケーションを妨げる要因の検討を行った。その結果、指示内容や周囲の状況が注射業務における確認行動に影響を与えていることが明らかになった。コミュニケーションエラーを防ぐには周囲の状況を変える必要があり、指示変更理由がチーム内で瞬時に共有するための手段、変更理由を尋ねやすい環境作りをチーム内で検討することが重要と思われた。2.患者-医療従事者間コミュニケーションの実態とあり方に関する研究薬害HIV感染被害者遺族約30名に面接調査を行い、患者告知を巡る医療従事者との関係について分析した。医師は患者に告知をせずに、患者との関係を、Awareness contexts theoryの、Closed awareness, Suspicion Awareness, Mutual pretense awarenessであることを望んだが、家族は患者との関係をOpen Awarenessであることを望んだ。その理由は、この疾患が医原病であり、患者に真実を隠すために家族が医療従事者と協働することができなかったことによると思われた。3.医療現場環境とエラー発生に関する研究注射業務のエラー発生に影響を与える業務特徴を明らかにするために、看護師、薬剤師に行った連続観測法タイムスタディー、ビデオ撮影分析を行った。注射業務を薬剤師は一カ所で同一業務を中断されない環境で行っているが、看護師は病室等を移動しながら、多重業務、様々な中断がある中行っていることが数量的に明らかになった。看護業務の特性上、多重業務をなくすことは困難であり、最終指示書に基づく注射薬の準備は他の部門や場所で行われることが、エラー防止には有効であることが明らかになった。
