项目摘要

S県H市における国民健康保険被保険者の受診状況を診療報酬請求明細書(以下、レセプトとする)を主たる情報源として検討した。レセプトに記載された情報を取り扱う上では、電算化されたレセプトデータを研究者が取り扱う前に、保険者であるS県H市の側で被保険者名および生年月日を削除し、さらに被保険者記番号などの個人同定に結びつく情報を変換する作業を加えることで、S県H市職員以外の者が単独で特定の個人を明らかにすることは不可能なデータセットを構築して個人情報保護の徹底を行った。また、集計の結果は保険者が実施する保健事業の評価指標の一部として還元する予定である。平成14年10月から同15年7月診療分の医科レセプト219,162件について、被保険者記番号からS県H市の側で作成した被保険者単位集計用の一連番号を用いて被保険者単位に各月毎の受診件数を集計した。医療機関の種類や診療科を問わず、月3件以上のレセプトが認められた被保険者を多受診者と定義し、その推移を年齢階級別に検討した。また、平成15年5月時点の被保険者数を分母として、被保険者総数に占める多受診者の割合の推移を平成15年1月1日時点の年齢階級別に検討した。被保険者全体に占める多受診者の割合は平成14年10月の12.1%が最高で、最低は平成15年2月の9.4%であった。平成14年10月から同15年2月にかけて減少し、それ以降は増加に向かう傾向が認められた。いずれの月も70-79歳の年齢階級で多受診者の割合が最高で、10-19歳の年齢階級が最低であった。0-9歳の小児についても毎月2-3%の多受診者が存在し、多受診者の存在は高齢者だけではないことが明らかになった。70歳以上の高齢者では平成14年10月から同15年2月にかけて減少し、それ以降は増加に向かう傾向が認められたが、0-9歳の小児については季節変動は明らかではなかった。高齢者における多受診者の問題を検討する上では季節変動を考慮した対応が重要と考えられた。
