介護保険制度では、保険財政の安定化を狙いとして複数の市町村が連携して保険者となるケースが多い。しかしながら、本来は域内でのサービス基盤の分布などに関する地域格差ゆえにサービス利用の活発さが市町村によって異なる状況にもかかわらず、保険料を構成市町村間で均一化させることは、負担と給付に関する公平性をかえって損なう逆差別的な影響も無視できない。本研究ではこうした点をふまえて、複数の市町村で構成される広域保険者における市町村別の負担と給付のバランスについて、全国初の試みとして地域ごとに複数の保険料設定(いわゆる保険料の不均一賦課)を実施している沖縄県介護保険広域連合を事例としてその意義を検討した。沖縄県介護保険広域連合は、県内52市町村のうち34市町村から構成され、第1号被保険料(標準額)は市町村によって3つの水準(第1ランク:3,217円7町村、第2ランク:4,333円11町村、第3ランク:5,225円16市町村)に区分されている。2004年8月時点でのサービス費用額および第1号被保険者数等のデータから、各市町村が独自に保険者となる場合の仮想単独保険料を求め、それを上記の保険料と対比したところ、少数の例外を除いて概ね85%〜130%程度の範囲に該当した。これに対して、広域連合全体で均一化した場合の保険料と対比すると、単独で設定した場合の保険料より実際には相対的に高い水準の保険料を賦課される自治体の数が多いことが明らかとなった。34市町村間のばらつきを変動係数で把握すると、前者の場合が0.178、後者が0.218であり、サービス給付実績と保険料水準との対応関係における広域化の逆差別的な影響に対して、不均一賦課によって一定の緩和的効果を持つことが確認された。ただし実際の保険料水準(3ランク)と比較して、後者の値や単独保険料水準の順序が逆転している場合が多く見られるなど問題も残している。
