在宅福祉の充実を理念とする介護保険制度が2000年4月から施行されたが、特別養護老人ホームなど施設福祉への需要が大幅に増加していると言われている。従来の研究から、特別養護老人ホームヘの入所に関する空間的移動は、1990年代以降の施設拡充策によって新しい施設が次第に建設される中で、従来の居住地など、より近接する施設への志向が強まった。本研究では特別養護老人ホームの入所者に関するアンケート調査を通じて、施設選択をめぐる意思決定や選好の実態を把握し、今後の施設立地のあるべき姿について検討することを目的とした。研究対象地域として、岐阜県の東濃老人保健福祉圏域に立地する特別養護老人ホームを対象にアンケート調査を試みた。13施設中9施設から協力が得られ、施設を通じて入所者の家族等身元引受人の方へ調査票を送り、郵送回収した。有効回答数は250(回収率44.6%)であった。主な回答者は息子(36.4%)、娘(20.0%)、嫁(17.2%)であり、入所施設の主な選択者は子とその配偶者が全体の54.4%を占めた。入所前の前住地は施設のある市町村内が52.4%で、市町村内を含む圏域内では80.0%となる。入所前に病院または老人保健施設へ入っていた入所者は78.8%に上り、在宅福祉サービスの利用経験者数(69.2%)を上回る。入所時の施設選択の主な理由は、入居者の自宅から近いこと、市町村役場のすすめ、他の施設より早期に入所可能であったこと、であった。これに対して、回答者が将来入所すると仮定した場合は、施設選択の主なポイントとして、職員の人柄など施設のソフト面が最も重要で、次いで施設のハード面、自宅からの近接性、家族の自宅や職場からの近接性が挙げられた。また、入所施設の所在地は、「前住地と同じ市町村内が良い」とする回答が62.0%を占めるなど、全体として近接志向が裏付けられる結果となった。
