项目摘要

第一線看護管理者が直面する看護管理上の倫理的ジレンマとその様相、および対処法を質的帰納的に明らかにするため、インタビュー調査を実施した。調査に先立ち、インタビュー内容と方法を検討するために、1990年以降の医学中央雑誌およびCINALにて、「中間看護管理者」「看護師長」「看護管理」「倫理」「ジレンマ」「意思決定」をキーワードに文献検索を行った。この結果と、看護倫理に関する文献、看護者の倫理綱領(日本看護協会,2003)を参考に、看護管理上の倫理的ジレンマに関連すると考えられたテーマ9項目を設定し、それらの項目の中で実際にジレンマとして体験したものについて、その状況や思考の詳細を問うためのインタビューガイドを作成した(インタビューガイドは調査実施途中で適宜修正)。調査協力の依頼は、病院長および看護部長から調査許可を得たのち、看護師長へ個別に依頼文書を送付、郵送にて承諾可否の返答を得る形で行った。また、インタビューの実施にあたり、対象者には前もってインタビュー内容の項目を送付し、ジレンマ体験を具体的に思い出しておいてもらうよう依頼した。現在までに、6総合病院29名の看護師長から協力を得ることができ、インタビュー時間は25分から89分、平均49分であった。平成16年度は、このインタビュー内容を看護管理研究者および臨床看護管理者のスーパーバイズを受けながら分析し、看護管理実践上の倫理的意思決定ガイドラインを作成する予定である。
