本研究は、看護職トップマネジヤーが、倫理的ジレンマが生じた際にも善しと思える意思決定を行い、倫理的風土が確立された組織作りを担えるためのモデルを提案することを念頭においている。その初段階として、1)実際に生じている倫理的ジレンマにどのようなものがあるのかについて現状を調査し、記述すること、2)倫理的ジレンマを生じさせている要因を明らかにすることを目的とした。第一次調査(平成12年11月から12月)では延べ11人、第二次調査(平成13年12月から平成14年2月)では8人の看護部長に面接調査を実施した。面接協力者の選定は研究者のネットワークを通じて行った。また、兵庫県立看護大学の研究倫理委員会に諮り研究倫理には十分配慮して実施した。面接では事前に「これまでに行った倫理的意思決定のうちで最も困難であった事例」について自由に語っていただいた。その結果、(1)日本的文化規範に従う、(2)住民の要望に応える、(3)政策的な要求を受け入れる、(4)個人の誇りを守る、(5)市民としての義務を果たす、(6)女性性を受け入れる、(7)社会的に人を助ける、(8)看護。の質を保証する、(9)患者の権利を守る、(10)患者の生命を守る、(11)組織の利益を上げる、(12)労働者の権利を守る、(13)看護部門を代表する、(14)医師と協調する、(15)組織のルールに従う、(16)法を守るの合計16の道徳的要求が衝突して倫理的ジレンマを引き起こしていることが明らかになった。また、ジレンマが生じても意思決定の際の価値基準や判断基準が明確である場合には、状況に左右されずに「善し」と思える意思決定が行えていることが示唆された。今後は、1)面接を重ねて道徳的要求のカテゴリーの理論的飽和をめざす、2)看護管理者の倫理的ジレンマを道徳的要求の葛藤パターン毎にまとめ、それらが日常的に起きる頻度を確認する、3)16の道徳的要求の影響の強さを探索する、4)道徳的推論を探索する、5)意思決定をサポートする仕組みを考案することなどを課題としたい。
