The relationship between cholesteryl ester transfer protein(CETP) and coronary heart disease is controversial. In populations with a higher serum high-density-lipoprotein cholesterol(HDL-C) level such as in Japan, blood CETP level and its effect on coronary atherosclerosis may be different from those of US populations. No previous study has compared CETP and its effect on atherosclerosis between Japanese and US whites. In 2002-2005, random samples of community-based 249 Japanese in Kusatsu city, Shiga and 233 US white men in Alleghency County, Pennsylvania, aged 40 to 49 were examined for coronary artery calcium(CAC) determined by electron-beam computed tomography and serum CETP concentrations. Mean CETP concentration is significantly lower in Japanese than in US(2.24 mg/liter(standard deviation, 0.45) vs. 2.43 mg/liter(standard deviation, 0.49)). Serum CETP was positively associated with CAC in Japanese men after adjusting for other cardiovascular risk factors(Odds ratio(OR) for a 1mg/liter difference in CETP, OR ; 4.27, 95% Confidence interval(CI) : 1.59, 11.5). This relationship was not affected even after adjusting for low density lipoprotein cholesterol and high density lipoprotein cholesterol. Serum CETP was not associated with CAC in US white men(OR, 0.79, 95% CI : 0.40, 1.56). This findings warrant further studies.
