项目摘要

最近の医療環境は大きく変化してきており、医療現場では情報の電子化により電子カルテが導入され、PHSなどのスタッフ間の情報伝達手段の変化や分散型の看護拠点が出現している。これにより、看護ステーションで行なわれていた看護記録の記載や医師からの指示受けなどは、集中化をしなくても可能となってきている。そこで本研究は、病棟環境の中でも看護拠点に注目し、看護体制および情報環境との関連性を明らかにすることを目的とした。本年度は、分散型看護拠点を設置し、チームナーシングを行っている首都圏のA病院2病棟での調査において、看護必要度と各病床から看護拠点までの距離および病床の種類との関連について詳細に分析を行った。その結果、両病棟ともにモニタリングや処置の高さを表す看護必要度Aの値は、看護拠点までの距離に相違がなかった。このことから、情報が電子化され、分散した看護拠点がある病棟において、患者のモニタリングは看護拠点からの距離を意図しなくてもよいことが明らかとなった。従来の小規模看護チームは病棟の物理的環境によって、十分機能しないと考えられていた。しかし本研究の成果から、分散された看護拠点があり、情報が電子化され、そこで情報を入手することが可能であれば、モニタリングや医療処置の必要性のある患者に対する看護ケアにおいては、小規模な看護チームでも可能であることが示唆された。しかしながら、日常生活援助の必要性のある患者に対する看護ケアの提供に関しては今後も検討が必要である。
