2008年1-7月の7ヶ月間,身体障害児とセラピストによる1対1の訓練セッションをデジタルビデオカメラにて撮影,データを収集し,2歳男児を対象とした事例研究を行った.まず,リーチング動作・対象操作行動の出現過程の分析から,麻痺側上肢の運動出現前に情動の安定が見られる可能性が示唆された.また,観察開始からおよそ3ヶ月後に,セラピストが設定する課題を変えたセッションにおいて対象児の自発的リーチング,ならびに対象操作行動がみられたことから,課題設定が運動において身体的制約をもつ身体障害児の自発的運動を促進することが推察された.対象児は観察以前にリーチングに必要な身体的要素(視覚機能,上体の支持,上肢の運動機能)を獲得していたにもかかわらず,リーチンクや対象操作行動がみられなかったことを考慮すると,セラピストの介入が対象児の身体的要素と相互作用し,自発的な上肢の運動を産み出したと考えられる.自発的な運動の発現とともにポジティブな情動反応,課題継続時間が増加していたことから,運動発達支援は,運動を学習するだけでなく,他者との社会的相互作用をとおした障害児の社会発達支援を包含していると考えられる.本研究の結果から,身体障害児の発達が(1)運動と情動,セラピストあるいは課題,といった個体内の要因と環境との相互作用によって促進されること,(2)運動をとおした発達支援は,社会発達を導く可能性が示唆された.
