Preeclampsia, characterized as hypertension and proteinuria threatens to both maternal and fetal life. It causes to increase vascular tone and permeability due to the activation of endothelial function. There are two differ types of the disease. One is dependent on the placental disorder, which is caused by faulty of placentation at early pregnant time. The increases in production of cytokines and superoxide, and insulin resistance might be involved in the pathophysiology with and without placental disorder. Thus, it is important for the management of diet, including sodium and vitamin to treat and prevent it, being similar to metabolic syndrome. In the present study, we investigated whether or not an oral supplementation of L-arginine plus folic acid could improve endothelial dysfunction seen in preeclampsia.The concentration of serum L-arginine had been increasing advancing gestational ages, however it was reduced in preeclamptic women. Furthermore, flow mediate vasodilatation (FMD), non-invasive evaluation of endothelial function using ultrasound was reduced in both preeclamptic and high risk women (previous preeclampsia). In preeclamptic women, the reduction could not be improved by profuse supplementation of L-arginine 9g plus folic acid 5 mg / day for a several days. In high risk women, the reduction could be improved by the profuse supplement as well as contentious administration of L-arginine lg plus folic acid 0.8mg/day from the second trimester. In conclusion, the supplementation of L-arginine plus folic acid might prevent the endothelial dysfunction seen in preeclampsia.
