認知症や昏睡、脳卒中後遺症などで患者が医療同意困難な状態になった場合、日本だけではなく欧米でも通常、家族などの患者の近親者が代理人となって意思決定を行っている。しかしながら、患者本人と代理人の意向の一致率はそれほど高くないことが繰り返し報告されている。本研究では医療同意困難な患者のより良い治療決定を検討するため,療養病床の実態調査,および一般住民の意向調査を行った.両調査において,患者の意向を尊重する上で重要なのが,家族をはじめとした重要他者とのコミュニケーションであることが示された.患者本人が自身の方針を実際に実現する上では,単に意向を表明するだけでなく,重要他者とコミュニケーションを行うことが重要である.また医師側の視点からは,患者の意向を尊重した方針を選択する上で,家族とのコミュニケーションは重要な要素であることが示唆された.医療同意困難な患者においては,重要他者も含めて問題点を共有し,患者の立場を尊重するコミュニケーションを行うことが重要であると思われる.
