本研究は、患者中心のエンド・オブ・ライフケアを提供するために、生を脅かす病いを持つ患者の死のあり方や死にゆく過程に対する考えを明らかにすることを目的とした。本年度は、看護師がGood Deathについてどのように考えているかについて調査を実施した。まず、Steinhauserが作成した調査用紙により39名の緩和ケアに携わる看護師に死に臨む際に重要な事項について質問紙調査を行った結果、全員が重要だと回答した項目は、「話を聴いてくれる人がそばにいること」「息苦しさがないこと」「尊厳が保たれていること」「家族がそばにいること」「親しい友人と共に過ごすこと」「大切な人に別れを言うこと」「痛みがないこと」であった。また、重要ではない卜回答した項目は「回復の可能性がなくても可能な限りの治療を行うこと」「聖職者に会うこと」「祈ること」「神と共に平和であること」であった。さらに計13名の緩和ケアに携わる看護師に対して、今までの看取りのなかで、満足した看取りと心残りだった看取りについてフォーカスグループインタビューを実施し、結果をデータ化し質的に分析した。その結果、看取りの質を構成する要素として、「苦痛から解放されていること」は基本的な要素として抽出され、さらに「患者・家族が納得していること」「医療者の誠心誠意な態度」がそれを補完する大きなカテゴリーとして抽出された。その他「おだやかな死に顔」「家族がそばにいること」「看護師自身の達成感」などが抽出された。今後は、Good Deathを構成する要素の関係性や、日本におけるGood Deathの概念について整理し、患者を対象とした調査を実施することが課題である。
