N-acetyl-β-D-glucosaminidase (NAG)とは、リソゾーム中に含まれる加水分解酵素の非還元性glucosidaseの一種で、体内組織に広く分布するが、血管内皮にも分布している。分子量が比較的大きいため、血清中のNAGは尿中にほとんど排泄されず、腎尿細管や糸球体障害で尿中に出現する。そのため、尿中NAG活性は腎病変の早期発見に有用な指標として広く臨床で使用され、知られている。Belfioreらは動脈硬化性疾患に血清NAG活性が高いことを報告(Am J Med Sci. 268 ; 235-239, 1972)しており、わが国における脳血管疾患の有病率・死亡率は高いため、脳・心血管病に焦点を絞り血清NAG活性との関連を研究・解明する。血清NAG活性を測定することで今後の医療現場で脳・心血管病の発症予防に寄与する-尿中NAG活性の臨床的意義に負けない-基礎資料を得ることを目的とする。横断研究の結果、昭和57年に田主丸住民検診を受診した1080名に血清NAG活性を測定しており、この分布に性差を認めなかったが、年齢とともに増加し、血圧(p<0.001)総コレステロール(p<0.001)、尿酸(p<0.001)、肥満(p<0.05)と正に関連することを認めた。また、7年間の追跡研究の結果、血清NAG活性が高値を示した正常血圧者は、将来高血圧に進展する頻度が高いことを我々の研究で報告している(Hypertension. 25 ; 1311-1314, 1995)。わが国では人口の高齢化、食生活の欧米化、運動の減少に伴い、生活習慣病は急速に深刻化している。血清NAG活性はこの生活習慣病:脳・心血管病と密接な関連を有することが考えられる。この検診地区は脳・心血管病死がガン死に比べて少ないため、今回有意な結果は得られなかったが、追跡年数を延ばし、研究解析を継続して行い血清NAG活性と脳・心血管病発症との関係を解明する予定である。
