项目摘要

本研究では、新たな「フィールド栄養学」として、既存の栄養学的アセスメント法に独自に開発したFDSK-11（食事の質の調査）を加え、地域の環境・文化的背景をふまえた「複合型調査モデル」を適用し、ヒマラヤ高地に住む人々の食の現状と環境・生活・文化、及びその変化を多面的に把握することによって、それが地域の生活習慣病の発生にどのように関連しているかを明らかにすることを目的とし、調査を進めてきた。調査地は、ヒマラヤ高地のラダーク地方の中でも、都市部のレー、山村部のドムカル、という２つの異なる生態環境や近代化の状況を背景をもつ地域において、各地の食事摂取状況と生活習慣病との背景を調査した。この調査には、新しい「フィールド栄養学調査」の手法を用い、従来の栄養・医学調査だけでなく、地域の環境・文化的背景をふまえた複合的な調査を行い、ヒマラヤ高地に住む人々の食の現状と変化を多面的に分析した。これらのデータを分析した結果、糖尿病の改善が見られていた患者はドムカルで3名、レーで2名にみられた。病気の診断後にどのような生活変化があったかについても、質的インタビューを行った。調査データをまとめると、生活習慣病の背景に、①食事内容の変化：大麦（ツァンパ）から米へ、②食の偏り（食多様性の乏しさ）があったといえる。食資源に乏しい標高3000mを超える高地に暮らす人々は、家畜や交易などから得られる限られた食品を様々に加工する独自の伝統的食文化を形成していた。そのしかしながら近年になって、標高4000mを超える高地にも道路ができ、低地や都市からの食材（野菜や果物）が流通するようになった。伝統的な主食であった大麦から米への転換が起こっている背景には、政府による安価な米の配給の普及があった。
