项目摘要

児がNICUに入院している褥婦が、産褥早期に頻繁な搾乳を継続して行えるよう支援する方法として、足湯に着目した。NICU入院児は、吸啜力の未発達や治療などのために出生直後から頻繁に吸啜による乳児嚥下ができない。そのため、母子分離状況下にある褥婦は、直接授乳の開始まで長期にわたり、搾乳により乳汁分泌を維持することが余儀なくされる。今回、対象の褥婦への足湯による乳頭・乳輪・乳房の状態変化を定性および定量的に評価する手法を確立するとともに、搾乳に及ぼす効果の分析を目的として研究を開始した。研究を開始するに当たって所属大学および研究機関の倫理委員会の承認を得た。2016年11月、データ収集を開始した。対象者数が不足したため、当初計画の期間を変更し、2019年3月末まで１年間延長した。実験期間中並びに実験終了後を通じて有害事象は起こらなかった。2016年11月から2019年3月までにA病院にて経膣分娩で出産し、児がNICUに入院している褥婦30名を対象に、実験群と統制群各15名の2群間比較を行い、結果をまとめた。足湯を実施した実験群は、足湯を実施しない統制群と比べて、乳頭および乳輪が伸び、かつ柔らかくなる結果が得られた。乳汁分泌量は、「射乳」の状態である実験群の方が統制群より多かった。さらに実験群は、硬結がみられなかった。搾乳に伴う疲労感などの不快症状を軽減し、快適感の上昇をもたらす結果が得られた。2020年度、日本母性衛生学会、日本看護学会、日本看護科学学会、日本助産学会で成果を報告した。2021年度、日本実験力学会、日本母乳哺育学会、日本看護学会（2回目）で成果を報告した。また、2021年度、母子分離状況下にある褥婦への足湯が子宮の復古に及ぼす影響について、愛知医科大学看護学部紀要に発表した。2022年度、皮膚ひずみ測定による褥婦の乳房緊満評価について、実験力学に発表した。
