Glypican3(GPC3)は膜結合型Glypicanファミリー6種類のうちの一つで1997年にHsuらがHCC組織においてGPC3の発現が有意に高いことが報告した。以後、卵巣癌、悪性中皮腫、乳癌、大腸癌などにおいても発現があることが報告された。小児固形悪性腫瘍に関してはいくつかの報告が散発的にあるが、その腫瘍マーカーとしての有用性までには言及されていない。今回我々はウイルムス腫瘍をはじめとする小児固形悪性腫瘍全般におけるGPC3の発現を検討し、新規腫瘍マーカーとしての有用性を検討した。方法は血清中のGPC3(sGPC3)、AFP(sAFP)、組織は免疫組織によりGPC3(tGPC3)、AFP(tAFP)について検討した。34例中3例がsGPC3(+) /sAFP(+)(肝芽腫2例、卵黄嚢癌1例)であった。一方4 例がsGPC3(+)/sAFP(-)であった(未分化肉腫1例、神経芽腫1例、Wilms腫瘍1例、 Solid Pseudopapillary Tumor1例)。このうち、未分化肉腫の症例においてはsGCP3が治療効果に伴い、経時的に減少傾向を示した。これらの所見よりGPC3はAFPに類似した動態を示す癌胎児性蛋白であるが、GPCを特異的に発現する小児腫瘍が存在することが示され、sGPC3(+) かつsAFP(-)を呈する腫瘍においては新規腫瘍マーカーとなる可能性が示唆された。
