项目摘要

日本の在宅の虚弱高齢者に適用する住まいの評価の標準化と,訪問リハビリテーション(訪問リハ)において活動参加の水準のゴール設定に用いる面接評価の適応の調査を行った.住まいの評価の研究では,Westmead Home Safety Assessment日本語版の検者間信頼性を検証した.日本には利用者と住まいの適合状況を専門職が評価する標準化されたツールがないので,海外で検者間信頼性が検証され,評価基準が「転倒の危険性の有無」だけであり構造が単純な,スクリーニング評価でない本評価を採用した.原著者の許可を得て,日本人に適用するために12項目に軽微な変更を加えた.65歳以上の高齢者32名の自宅を2名の作業療法士が訪問して,独立に評定した.対象は平均年齢78.4±7.5歳,女性が18名,屋内独歩の者が18名であった.72項目中53項目(74%)では,中程度から良好以上の信頼性(κ>0.4)が認められた(p<0.05).不良な信頼性(κ≦0.4)の項目はなかった.他の19項目では危険性有りの評定が少なく,標準化しない参考項目として残すかどうかを検討する必要が認められた.活動参加の評価の研究では,COPM(カナダ作業遂行測定)を訪問リハの実践で使用した評価者のフオーカスグループを実施した.訪問リハ開始時から活動参加の目標の明確な人,情緒的に安定した人,理屈を好む人では実施しやすく,活動参加のゴールの設定や達成に有効であることが示唆された.本研究でけ,訪問リハで問題解決型チームアプローチを実施するツールとして,住まいの評価を開発し,活動参加のゴール設定のための評価の適応を示した.本研究で扱った構造化された2種類の評価の普及は,実践上の問題解決に加えて,訪問リハビリテーション分野の課題の体系的抽出およびプログラムの適応有効性の検証といった科学的基盤の構築にも役立つことが期待された.本研究でけ,訪問リハで問題解決型チームアプローチを実施するツールとして,住まいの評価を開発し,活動参加のゴール設定のための評価の適応を示した.本研究で扱った構造化された2種類の評価の普及は,実践上の問題解決に加えて,訪問リハビリテーション分野の課題の体系的抽出およびプログラムの適応有効性の検証といった科学的基盤の構築にも役立つことが期待される.
