项目摘要

本年度はドイツキール大学心臓血管外科、Prof.Lutterのチームと共同で僧房弁位におけるナイチロールステントのデザインを改良し、経カテーテル的僧房弁置換術を行った。術後1ヶ月の評価を行った。今回使用したステントは心房部が直径58mm、心室部が直径28mmで45度の角度をなすものを使用した。ステント周囲は防水性生地で被覆した。生体弁には牛心膜性弁を使用した。成豚45～55Kgを使用し、胸骨下部を切開。心臓を露出し、心尖部より、人工心肺を使用せず、心拍動下に生体弁付ステントを留置した。術中、術中急性期の評価を行い、4週間生存させ、経食道心エコー、CT検査、左室造影検査を用いて、ステントの位置、弁周囲の逆流の有無、僧房弁流入波形の測定、左室流入路の圧較差の測定を行い、1ヶ月後に心臓を摘出し、肉眼的、顕微鏡的評価を行い、ステント破損の有無、心筋損傷の有無について、ステント固定法について術後1ヵ月の評価に取り組んだ。その結果、術後1ヶ月の評価ではCT検査において、ステント位置、固定は良好であった。経食道心エコー検査では生体弁は良好機能し、左室流出路狭窄は認めず、血行動態は安定していた。術後1か月後の摘出標本では、ステントの破損、心筋損傷は認めなかった。また、心房部のステント部における内膜被覆率は術後1ヵ月生存の心臓では50%、2カ月生存の心臓では70%にわたっていた。血栓塞栓症の所見も認められなかった。これは、経カテーテル的僧房弁置換術が従来の人工心肺を使用し、心停止下に心臓を切開して人工弁を取り付ける弁置換術では耐術困難な複数の合併症を持つ重症弁膜症患者にとって従来弁置換術にかわる低侵襲な治療法の一つになりうる可能性を示している。実際に大動脈弁、肺動脈弁にいては臨床応用されているが、これらの治療法が解剖学てきに異なる房室弁においても可能であることを示したことに意義のあるものである。無論、さらなる改良、長期的評価が必要である。
