项目摘要

高齢者では、嚥下機能の低下が誤嚥性肺炎や窒息など生命に関わる問題を引き起こす. 高齢者の嚥下障害は、廃用萎縮により、修飾・助長されているといわれている. 特に食事に嚥下動作を必要としない胃瘻症例や、唾液を嚥下する機会が少なくなるドライマウス症例では、嚥下回数の減少は顕著であると考えられる. 我々は、日常生活での嚥下回数を測定することにより、年齢や症状にあわせた標準値を決定することおよび、嚥下機能の廃用萎縮予防の訓練を行なうための嚥下回数の指標となるものを確立することを目的に研究を行ない、以下の結果を得た. 《健常成人において》1)1回の食事における平均嚥下回数 : 56.2±10.7回2)ビデオ鑑賞時 : 40.3±19.9回/時間《要介護高齢者において》1)睡眠時7.6±19.7回/時間2)ビデオ鑑賞時 : 8.8±3.7回/時間(健常成人と比較して有意に低い値を示した)3)誤嚥性肺炎の既往がない群 : 12.3±7.4回/時間, ある群 : 9.0±5.2回/時間(両群に有意差の差は認められなかった.)《唾液分泌量について》1)ドライマウス症例 : 5.6±6.8回/15分, Age matchingをした健常成人 : 11.2±7.6回/15分、(両群に有意の差は認められなかった.)2)唾液分泌量と嚥下回数の相関を検討した結果、正の相関が認められた. 本研究の結果、要介護高齢者では嚥下頻度が低下しており、唾液の分泌量も嚥下頻度の低下の原因の一つとなる可能性が示された. この嚥下頻度の低下により嚥下機能の低下をまねくのか、もしくは、反対に嚥下機能が低下しているから嚥下頻度が低下していたのか因果関係は不明である. 今後、嚥下機能の評価と嚥下頻度の比較および長期的な嚥下頻度の計測を行なっていき、因果関係を明らかすることで、高齢者の嚥下頻度のリハビリテーションメニューの決定の一助になると考えられる.
