1歳6カ月、2歳6カ月健診時にあらかじめ養育者にセルフコントロール、注意、睡眠、食事、触覚、運動、聴覚・言語、愛着・情緒など乳幼児期の困難さを広範囲にチェックできるITSCを記入し持参してもらうことで、健診場面で養育者の子育ての困難さに、より具体的に、迅速に対応する取り組みを行ってきた。本研究ではその効果を検討するために(1)3歳以降のADHDや自閉症スペクトラムなどの発達障害とITSCチェックリスト評価がどのような関係にあるのかを分析、また、(2)ITSCチェックリストを健診場面で使うことへ養育者がどのように感じているのかをアンケートにより分析した。464人のITSCチェックリストを因子分析した結果、チェックリストの項目区分とは必ずしも一致せず、セルフコントロール、感覚運動、対人・コミュニケーションなどの因子が抽出された。対象児で4歳半までに何らかの発達障害の診断を得たのは全体の約5%であったが、更に約9%がグレーゾーンに位置すると判断された。中で最も多かったのは自閉症スペクトラムであった。この自閉症スペクトラムと優位な相関がみられたのは対人・コミュニケーション因子であった。養育者アンケートは(1)ITSCチェックリストの構成(2)健診におけるITSC活用の効果(3)子育てへの負担感についての質問から成っており、176名から回答を得た。ITSCの内容や構成に関しては概ね適切との結果であった。ITSCの健診場面での活用に関しては70%前後の養育者がチェックリストがあることで子供の様子が伝えやすく、また、健診場面で支援を得るのに役に立ったと回答した。子育てへの負担感については、13%の養育者が何らかの育て難さを抱えており、40%の養育者が子どもの発達について悩みを持った経験があったと答えた。以上、ITSCチェックリストは自閉症スペクトラムの早期発見に役立つとともに、養育者が健診場面で相談をする上で有用であると考えられた。
