今年度は,小児がん患児における骨髄穿刺等の侵襲的な医療処置の前後の唾液アミラーゼ活性値と行動観察スケールを用いて評価した患児のdistress,患児および保護者が評価した医療処置に対する患児の不安,医師が評価した医療処置中の患児の身体的苦痛の程度の関連性について検討を行った。その結果,担当医師が評価した医療処置中の患児の身体的苦痛の程度と医療処置直後の患児の唾液アミラーゼ活性値の間に,有意な正の相関関係が認められた。医療者は患児のdistressの程度を患児の様子や行動に基づいて客観的に評価する傾向があることが報告されており,唾液アミラーゼ活性値が侵襲的な医療処置に際する患児のdistressを反映する客観的指標として有用である可能性が示唆された。近年,わが国においても患児のストレス(distress)を予防・軽減するための心理学的介入の重要性についての認識が高まるとともに,医師や看護師,チャイルド・ライフ・スペシャリスト等による取り組みが増えており,介入効果を客観的エビデンスに基づき評価できる方法や指標が求められている。本研究で得られた成果は,携帯型測定機器を用いて簡便に測定できる唾液アミラーゼ活性値が,そうした指標の一つとなりうる可能性を示すものである。今後は,実際の介入場面における測定等を通して,その効果を判断する指標としての唾液アミラーゼ活性値の有用性を検討する必要があると思われた。

The tendency of the physician to evaluate the severity of the distress of the patient and the behavior of the patient based on the evaluation of the patient's behavior is reported, and the likelihood of the patient's distress being reflected during the medical treatment is indicated. In recent years, the importance of psychological intervention in the prevention and reduction of child distress in China has been highly recognized, and doctors and nurses, such as doctors and nurses, have been selected to increase the number of groups involved, and the methods and indicators for evaluating the results of intervention have been developed. The results of this study demonstrate the feasibility of using a portable measurement machine to determine saliva activity values and indicators. In the future, it is necessary to discuss the usefulness of the evaluation index and the evaluation result of the actual intervention situation.
