平成29年度は、最終年度であり、1.これまでの研究のまとめ、2.高齢者の医療利用の分析を主に行った。まず、1.これまでの研究のまとめとして、前年度から実施している医療費の自己負担が患者の医療利用に与える影響の分析に関する論文の投稿を進めた。これはHealth Economics Review誌に掲載された。この研究は、近年一部市区町村が急速に対象者を拡大させている子ども医療費助成制度の影響を見たものである。分析の結果、外来医療に対する自己負担の減少は低所得地域において入院件数を減少させる一方、高所得地域では入院件数を増加させていることが明らかになった。さらに、平成27年度から実施している医療機器の普及の分析をさらに詳細に行った。本研究は病院間の競争が医療技術の普及を進める効果があること、特にその効果は民間病院において強く確認されることを明らかにした。これは医療サービスの供給に民間病院が大きな存在を占める日本において重要な示唆を与えるものである。競争によってより効率化するという期待はあるものの、医療市場においては競争を促進することは高額な医療技術の普及をもたらし、必ずしも医療費の減少にはつながらない恐れがある。本研究はディスカッションペーパーとして公開する予定である。2. 高齢者の医療利用の研究では、医療と介護を包括的に捕捉する分析を行った。本研究ではある自治体の協力を得て、国民健康保険と介護保険のレセプト情報を提供いただいた。これを用いて、医療サービス利用の増加が介護サービス利用に与える効果を推定した。その結果、医療サービス利用の増加は介護サービス利用を減少させることが明らかになった。これは適切な制度設計によって、医療・介護を合わせた総医療・介護費をより適正化しうることを示唆している。この成果は医療経済学会、国際医療経済学会（iHEA）にて報告を行った。
