项目摘要

本研究は、これまで社会学的にも社会的にも注目されることのなかった「名づけられない疾患」、未確定希少難病患者へのインタビュー調査、共同生活を含むフィールドワークを通して、病いとの共生モデルを構築することが目的であった。本邦において患者会が組織されていない疾患を持つ患者への調査を行うにあたって、各患者会と指導教員の協力を得て、13名の未確定希少難病患者とその患者の家族や担当医を含めて周囲にいる人々、32名への調査を終えた。また、インタビュー調査・フィールドワークだけでなく、未確定希少難病患者が置かれている社会的構造を把握する必要があると考え、難病政策・難病法成立に関する審議録をのべ60件を分析した。難病政策・難病法の審議録の分析から、日本国内においては希少未診断患者をはじめ診断名を持たない患者への具体的支援は検討されていないのが現状であることが明らかになった。また、インタビュー調査・フィールドワークのデータを分析したところ、未確定希少難病患者は診断名が付与されないことによって、親をはじめとした周囲の人物に詐病を疑われ、患いながらも患者として休むことを許されない状況を長期間経験していることが明らかになった。この期間に患者は複数の医療機関を受診するが、診断名の確定までは至らず、ほとんどの患者が「先行症例のない新しい疾患である可能性」を医師から告げられる。そして、ドクターショッピング期間を経過すると、患者は診断名の探求をやめ、「今の自分にできる」生活を営むことに集中する。最終的には医療機関への受診や定期検診は必要最低限の回数まで減少し、症状の悪化によって就労が難しくなると障害手帳を取得するなど診断名に依拠しない支援を活用しながら症状との共生を強いられていることが明らかになった。
