项目摘要

少子高齢化が本格化する中、コンパクト・プラス・ネットワークの考えに基づき、都市構造の再編を推進するために、立地適正化計画制度が創設された。2021年7月現在、全国398自治体は計画を作成・公表している。その中で多くの自治体は、病院を都市機能誘導施設として指定している。また、国土交通省「健康・医療・福祉のまちづくり」ガイドラインでは、地域包括ケアシステムの構築とまちづくりとの連携等により、地域全体で市民生活を支えることができる都市構造への転換が求めれている。このような背景を踏まえて、本研究は少子高齢化社会における地域医療サービスの格差を是正するために、地域医療の需給バランスを明らかにし、医療効率の観点から医療施設の立地と移転の傾向を把握した上で、地域医療資源の再配分を念頭に医療施設の配置計画及び医療政策の転換に有用な知見を提示することを目的とする。近年、医療資源の効率的な配分を目的として、一か所に複数の診療所や歯科診療所、薬局が集まって開業している医療モールは増え続けていることを踏まえて、医療モールは一か所で複数の専門的な治療を受けることができ、また診療所間での連携がしやすいことから患者さんにとってのメリットが大きく、地域医療の核としての役割が期待されている。一方、比較的規模の大きい医療モールの開発及び出店は周辺地域への影響も大きい。そこで、本年度は医療モールに着目し、近年の出店傾向及び、出店による周辺地域への影響を明らかにするとともに、医療効率の観点から一般的な診療所と比較しながら、地域医療の核としての医療モールの役割を明らかにしている。
