近年,我が国は,世界の最長寿国として高齢者人口が急増し,成人病の中でも循環器系の疾患が極めて多くなっている。特に心筋梗塞や脳梗塞栓症などの血栓に伴う患者数は増加の一途をたどりつつある。このような疾患は,心臓疾患の僧帽弁狭窄症と関わりがあると言われているが,その発生のメカニズムは未だ解明されていない。そこで筆者らは,上記疾患の発症のメカニズムの解明と事前の診断方法の確立のために経食道心エコー図法(Transesophageal Echocardiography)によりデータを収集した。この方法は,食道内に超音波探触子を挿入し心臓部位の短軸断面像及び四腔断面像を得るもので,血液のよどみなども観測でき,血栓症の疾患者の場合,左心房内にモヤモヤした筋状のゆらぎ(Spontaneous Echo Contrast:モヤモヤエコー)が観測される。このモヤモヤエコーは,血流速度の低下した部位において生じ,血栓が形成されやすく,脳梗塞栓症と高い相関があるとされている。モヤモヤエコーの定量化の解析にあたっては,まず,得られたデジタルビデオ画像をキャプチャーボードを介してワークステーションに取り込み,画像の特徴を損なうことなく雑音成分を除去するためのFFT処理によって空間周波数スペクトルを求め,高次成分の除去を行った。赤血球凝集体の抽出は,左心房内の画像濃度のダイナミックレンジの正規化を図り,バックグランドとの画像濃度分布の差から求めた。また、前述のようにモヤモヤエコーは揺らいで観測されることからフレーム間(1/30秒)での連結性を考慮し,塊の平均値の時系列的な変化として抽出した。その結果,一心拍間の定量値の最大と最小の差は,医師の5段階評価と高い相関を示すという良好な結果が得られた。今後は、赤血球凝集体の位置や流動方向などを解析し,血栓症との因果関係を解明し,予後の診断支援に役立てたいと考えている。

The extraction of red blood cell aggregates, the normalization of the image concentration in the left atrium, and the difference between the image concentration distribution in the left atrium are calculated. In consideration of the connectivity of the above mentioned time series, the average time series of blocks is changed and extracted. The results showed that the maximum and minimum differences between the quantitative values of one heart beat were good, and the results of the 5-stage evaluation were good. In the future, the location and flow direction of red blood cell aggregates will be analyzed, and the causal relationship between thrombosis and thrombosis will be clarified.
