本研究の目的は、在宅人工呼吸療法中の難病療養者の安全な療養生活に向けて、保健所保健師が安全管理として取組むべき活動の実践状況を測定するための「在宅人工呼吸療法中の難病療養者への保健所保健師の安全管理自己評価尺度（以下、保健所保健師のHMV（Home mechanical ventilation）安全管理自己評価尺度）」を開発することである。令和４年度は、開発する尺度の概念の明確化及び尺度原案の作成、本調査を実施した。開発する尺度の概念の明確化では、「在宅人工呼吸療法中の難病療養者の安全管理における保健所保健師の活動」について、既存の看護モデル（「The Intervention Wheel」）を適用し、医療や看護分野で規定されている「安全管理」の定義を取り入れ、明確化した。次に尺度原案の作成では、構成概念に合致する保健活動を先行研究より抽出し、アイテムプール（約85項目）を作成した。その後、専門家会議、10人の専門家によるパイロットスタディを経て、51項目の尺度原案を作成した。専門家会議及びパイロットスタディの協力者には、難病保健活動の実践者及び研究者を中心に依頼し、現在の保健所保健師の安全管理活動の実状を反映した尺度原案になるように努めた。本調査は、全国の保健所保健師を対象に令和５年２月～３月にかけて実施した。本調査では、信頼性の検証のために、再テスト法を実施した。以上のような信頼性・妥当性の検証の過程を経ることで、信頼性・妥当性を確保した尺度開発が可能となる。また、信頼性・妥当性の検証を経ることで、最小限の質問項目で安全管理活動の実践状況が評価できる測定用具が開発できる。さらに、自己評価が、自身の活動を査定し改善する一連の行動であることから、本尺度の活用により在宅人工呼吸療法中の難病療養者への安全管理活動の改善が可能となる。

The purpose of this study was to develop a self-evaluation scale for safety management of health care workers in home mechanical ventilation therapy (hereinafter referred to as HMV (Home Mechanical Ventilation) safety management self-evaluation scale) to measure the safety management and group activities of health care workers in home mechanical ventilation therapy for patients with refractory diseases. In order to clarify the concept of the standard and the preparation of the original standard, the investigation was carried out in 2004. Clarification of the concept of development criteria includes the introduction and clarification of the definition of "safety management" in the application of "The Intervention Wheel" to existing nursing care facilities in the context of "safety management for patients undergoing in-house artificial respiration therapy." The first step is to develop a comprehensive health care plan (about 85 items). After the meeting, 10 people from the family, the original plan of 51 projects was made.
