本研究の目的は、訪問看護師がこのような急変対応スキルを獲得できる、遠隔での教育プログラムを開発することである。2022年度は、高齢在宅療養者の急変時における訪問看護師の判断と対応を明らかにすることを目的に研究を進めた。訪問看護師への面接調査にて、データを収集、質的に分析し、高齢在宅療養者の急変時における訪問看護師の判断と対応を明らかにすることにした。面接調査に先立ち、在宅療養高齢者において、救急搬送判断時の症状の特徴について明らかにする目的で文献検討を行った。医学中央雑誌にて「高齢者」「救急」「在宅」をキーワードに検索された、在宅療養中の救急搬送高齢者事例21例から、年齢・在宅療養の目的の疾患、救急搬送判断時に呈していた症状を抽出した。21事例について、年齢は60代後半5人、70代前半3人、70代後半5人、80代前半4人、80代後半5人であった。主疾患は、COPD・肺気腫が9人と最も多く、在宅酸素療法や陽圧換気療法をしていた療養者が15人と半数以上であった。救急搬送判断時の症状で最も多かったのは、意識障害と呼吸困難であった。胸痛・発熱・転倒・嘔吐などもあったが、少数であった。高齢者は、非特異的な症状を呈しその症状は多様である。このことから、意識障害と呼吸困難の症状に偏らないよう、「どのような情報から身体状態をアセスメントし、緊急度および救急搬送の必要性を判断したか」と幅広い視点が語れるインタビューガイドを作成した。
